Step 6 – If you have had a role change during the period you are self-assessing, note this under point 2. Also, provide a self-report on how you were able to adapt to the new job requirements imposed on you.

Step 8 – If you have any weaknesses that you believe have an effect on your work performance, you should report them in point 4. For example, if you are prone to hay fever, your work performance at an outdoor cafe it may depend on any allergy medicine you may need to take.

Step 9 – In point five, report any aspect or quality you possess that could have a positive effect on your job performance. For example, owning a show dog would have some obvious benefits for someone who works in a pet store.

Step 10 – The next step will ask you to report on what expectations, aspirations or concerns you believe apply to the next Assessment Period. A good example might include a sudden expansion of operations without adequate manpower for your department to handle it.

Step 11 – The final item will ask you to rate your overall job performance during the period you are evaluating. You will need to tick a checkbox to set this. The check boxes are labeled: Excellent, Exceeds Expectations, Meets Expectations, Below Expectations, or Unsatisfactory. Check the box that you believe determines your job performance during this evaluation period.

Step 12 – Review your answers. Make sure this is an honest self-assessment, then submit this form to the relevant Department or Office at your place of work.
